TEAM REGISTRATION FOR SPRING 2012 LACROSSE IS NOW OPEN

Register In 4 Easy Steps:

• Step 1: US Lacrosse
  Register your player with US Lacrosse by clicking on the following link: www.uslacrosse.org
  Player membership is mandatory in order to play for OZLAX.

**During the US Lacrosse registration process, you will be prompted to enter your zip code, please ‘affiliate’ yourself with our group by selecting Ozaukee Lacrosse from the drop down menu or by entering [Group ID 2556117]

• Step 2: Download Forms
  Download and fill out completely all required documents by clicking on the register for SPRING LACROSSE link at the bottom of this page.

• Step 3: Payment
  Registration fee can be paid in full or in 2 installments:
  1/2 due upon registration – This holds a spot on a team
  Balance due March 1st – No player participation until received

• Include check made out to: Ozaukee Lacrosse
  *No electronic payment is available at this time.

• Step 4: Mail
  Mail all completed forms with payment to:

• Beginners are encouraged to join. Teams are co-ed. We are an instructional and equal – playing time sports league- see Team Philosophy
• Teams are determined by grade level- Developmental K-1st; In-House 1st-3rd; 3-4th grade; 5-6th grade; 7-8th grade; HSJV 9-12th grades.
• Team placement: Determined by student’s current grade level at time of registration.
• Lacrosse is a Spring sport, practices will begin early to mid March.
• Practices: 90 minutes long, 2-3x weekly
• Days and Times: TBD for each team
• Game schedule: [2] times weekly
  [1] game [home or away] on Saturday;
  [1] game [home or away] on Sunday.
• Practice and Home field: Mequon Area, TBD.
• Registration fees: Download registration forms for cost breakdowns for each program.
  
• *the registration fee does not cover tournaments; Tournament team/s will be selected at the beginning of June- additional fees will be collected at that time.
  
• A valid US Lacrosse membership number is required to complete the registration process. US Lacrosse Membership Requirement [Mandatory]

Before you register your player with Ozaukee Lacrosse [OZLAX], please check the expiration date of their US Lacrosse membership at www.uslacrosse.org


*It is required that each player have a valid US Lacrosse membership that is current through June 31st 2012.


If the membership expires prior to the respective season’s expiration month, please renew the annual membership fee or the player will be ineligible to participate. Parents are responsible for a current membership number and validation in order to complete the OZLAX registration process. This membership provides the league with insurance for the team and requires 100% membership participation; a player cannot begin practice until we receive confirmation from US Lacrosse that the membership is current for the appropriate season.

Ozaukee Lacrosse offers families a discount for multiple players within a family. A $25 discount will be given for each child registered after the first registration. The discount can be applied during the registration process for the 2nd and 3rd children in the program within the same family.

Cancellation Policy

Registration fee is 100% fully refundable within the first 2 weeks of the start of the first practice.
